Lifestyle Questionnaire

 

Your caloric needs are determined by your  height and  weight as well as your age, sex and activity level. If you want to lose, maintain or gain weight we can show you how to do this safely.

However, before you make any dietary changes you should discuss this with your physician or dietitian if you are already working with one.

 

Choose  the  activity  level  that  best matches  your  lifestyle:

 

Sedentary means seated and standing activities such as driving a car and typing.

Light Activity refers to activities such as leisurely  walking, house cleaning and golf.

Moderate Activity includes heavy housework, brisk walking, gardening, dancing, cycling, tennis and very little sitting.

Very Active includes running, basketball, soccer and heavy manual labor.

The rule of thumb is that the average female needs approximately 1500 calories while the average male needs approximately 1800 calories daily. If you are very active you will need more and if you lead a rather sedentary lifestyle you will need less.


After deciding which activity level best describes your lifestyle you must then decide if you want to gain, lose or maintain your present weight.
